    RESEARCH ON FORMULATION AND EVALUATION OF INSITU MUCOADHESIVE NASAL GELS OF METOCLOPRAMIDE HYDROCHLORIDE

    ABSTRACT The prolonged residence of drug formulation in the nasal cavity is of utmost importance for intranasal drug delivery. The objective of the present investigation was to develop a mucoadhesive in situ gel with reduced nasal mucocilliary clearance in order to improve the bioavailability of the antiemetic drug, Metoclopramide Hydrochloride. The in situ gelation upon contact with nasal mucosa was conferred via the use of the thermogelling Methyl cellulose whereas mucoadhesion and drug release enhancement were modulated via the use of sodium alginate and polyethylene glycol polymers respectively. The results revealed that the mucoadhesive polymer increased the gel viscosity but reduced its sol gel transition temperatures and the drug release. The inclusion of polyethylene glycol polymer counteracted the effect of mucoadhesive polymer where by it decreased the gel consistency and increased the sol gel transition as well as in vitro drug diffusion. The in vitro tests performed for mucoadhesive strength and drug diffusion showed that nasal in situ gelling formulations prepared are having good mucoadhesive strength with nearly100percente drug diffusion within four hours. So this study points to the potential of mucoadhesive in situ nasal gel in terms of ease of administration, accuracy of dosing, prolonged nasal residence and improved nasal bioavailability.  Keywords: Nasal Gel, Metoclopramide Hydrochloride, Methyl Cellulose, Mucocilliary Clearance

    The 2017 Vermont Opioid Prescribing Rules: Prescriber Attitudes

    Introduction. In July of 2017, Vermont enacted new rules on acute opioid pre- scribing to reduce misuse, addiction, and overdose associated with prescription opioids. The new rules include requirements of non-opioid therapy use when possible, querying VPMS, patient education and informed consent, and co-prescription of naloxone. Our study objective was to gain insight into the perspectives of opioid prescribers on the new rules. Methods. The 17-item survey included closed and open-ended questions addressing prescriber perceptions about the new rules as well as demographic information about respondents. The survey was sent to Vermont-based opioid prescribers via email, to multiple healthcare organizations and professional societies, and through personal contacts. Open-ended responses were categorized using paired reviewers and group consensus, using a grounded theory approach. Results. A total of 431 responses were obtained, with MD/DOs accounting for 65%, APRNs- 14%, DDS/DMD- 7%, PAs-13%, and NDs- 1%. Of the respondents, 75% thought that more restrictive opioid prescribing rules were necessary, 74% felt the new rules would have some positive effect on the opioid crisis, but only 48% were in favor of the new rules. Barriers to implementation included co-prescribing naloxone (50% were unsuccessful), justifying exceptions to rules in medical record (46% unsuc- cessful), considering non-pharmacologic therapies (39% unsuccessful), and adhering to prescription limits (31% unsuccessful). Conclusions. Roll-out of the new rules has been criticized for implementation issues, overall reducing favorability among prescribers. Feedback obtained may be utilized by the Vermont Health Department and by other states to improve current models of opioid prescribing.https://scholarworks.uvm.edu/comphp_gallery/1264/thumbnail.jp

    Hubungan Antara Keterlibatan Mengikuti Kegiatan Ekstrakurikuler Dengan Psychological Well-Being Mahasiswa Emerging Adulthood Universitas X di Surabaya

    Psychological Well-Being adalah suatu kemampuan individu merealisasikan serta mengoptimalkan dirinya dengan cara menerima diri seutuhnya, memilki kemandirian dan tidak terpengaruh dari tekanan luar, mampu mengontrol lingkungan di sekitarnya, mampu membangun relasi sosial, memiliki tujuan hidup yang jelas, serta dapat merealisasikan potensi dirinya secara kontinyu. Salah satu faktor yang memengaruhi psychological well-being mahasiswa ialah keterlibatan mengikuti ekstrakurikuler. Penelitian ini bertujuan untuk menguji hubungan antara keterlibatan mengikuti ekstrakurikuler dengan pscyhological well-being. Pengambil sampel dilakukan dengan menggunakan teknik purposive sampling. Subjek penelitian ini adalah mahasiswa Universitas Surabaya sebanyak 192 mahasiswa (131 perempuan dan 61 laki-laki). Teknik analisis yang digunakan ialah uji hipotesis dengan Pearson. Berdasarkan hasil uji korelasi Pearson menunjukkan adanya korelasi siginifikan positif antara keterlibatan mengikuti ekstrakurikuler dengan psychlogical well-being (r = 0.154, p = 0.033). Sumbangan efektif yang diberikan keterlibatan mengikuti ekstrakurikuler dengan psychological well-being ialah sebesar 2%. Keterlibatan mengikuti ekstrakurikuler berkorelasi dengan aspek psychological well-being yakni aspek personal growth (r = 0.177, p = 0.014) dan aspek positive realtion with others (r = 0.0215, p = 0.003)

    A peer-reviewed eBook, which is based on a collaborative research project coordinated by Dr. Henrik Schoenefeldt at the Centre for Architecture and Sustainable Environment at the University of Kent between May 2013 and June 2014. This project investigated how architectural practice and the building industry are adapting in order to successfully deliver Passivhaus standard buildings in the UK. Through detailed case studies the project explored the learning process underlying the delivery of fourteen buildings, certified between 2009 and 2013. Largely founded on the study of the original project correspondence and semi-structured interviews with clients, architects, town planners, contractors and manufacturers, these case studies have illuminated the more immediate technical as well as the broader cultural challenges. The peer-reviewers of this book stressed that the findings included in the book are valuable to students, practitioners and academic researchers in the field of low-energy design. It was launched during the PassivHaus Project Conference, held at the Bulb Innovation Centre on the 27th June 2014

    Computational analysis of superfood representations in news media

    What do berries, avocado, quinoa, and ginger have in common? These food items are often regarded as superfoods, a marketing term that overstates the importance of single food items for one’s health and wellbeing. In the present paper, we set out to investigate how purported superfoods are represented in the discourse of online news. We use computational language models to extract the unique topics and terms used to discuss superfoods. Our results show that news coverage is dominated by many specific claims about the healing properties of superfoods. The structural topic model further demonstrates that articles mentioning superfoods are more likely to include topics about a) nutrients, physical appearance, and health in the same context, b) retail strategies, and c) scientific research about the health benefits of superfoods. These results illustrate complex representations of superfoods in news media

    Computational methods for predicting and understanding food judgment

    People make subjective judgments about the healthiness of different foods every day, which in turn influence their food choices and health outcomes. Despite their importance, there are few quantitative theories about the psychological underpinnings of such judgments. This study introduces a novel computational approach that can approximate people’s knowledge representations for thousands of common foods. We use these representations to predict how both lay decision-makers (general population) and experts judge the healthiness of individual foods. We also apply our method to predict the impact of behavioral interventions such as the provision of front-of-pack nutrient and calorie information. Across multiple studies with data from 846 adults, our models achieve very high accuracy rates (r2 from 0.65 to 0.77), and significantly outperform competing models based on factual nutritional content. These results illustrate how new computational methods applied to established psychological theory can be used to better predict, understand, and influence health behavior

    Diagnostic accuracy of a point-of-care urine tenofovir assay, and associations with HIV viraemia and drug resistance among people receiving dolutegravir and efavirenz-based antiretroviral therapy

    Introduction: Novel point-of-care assays which measure urine tenofovir (TFV) concentrations may have a role in improving adherence monitoring for people living with HIV (PLHIV) receiving antiretroviral therapy (ART). However, further studies of their diagnostic accuracy, and whether results are associated with viraemia and drug resistance, are needed to guide their use, particularly in the context of the global dolutegravir rollout. Methods: We conducted a cross-sectional evaluation among PLHIV receiving first-line ART containing tenofovir disoproxil fumarate at enrolment into a randomized trial in two South African public sector clinics. We calculated the diagnostic accuracy of the Abbott point-of-care immunoassay to detect urine TFV compared to liquid chromatography-tandem mass spectrometry (LC-MS/MS). We evaluated the association between point-of-care urine TFV results and self-reported adherence, viraemia ≄1000 copies/ml and HIV drug resistance, among people receiving either efavirenz or dolutegravir-based ART. Results: Between August 2020 and March 2022, we enrolled 124 participants. The median age was 39 (IQR 34–45) years, 55% were women, 74 (59.7%) were receiving efavirenz and 50 (40.3%) dolutegravir. The sensitivity and specificity of the immunoassay to detect urine TFV ≄1500 ng/ml compared to LC-MS/MS were 96.1% (95% CI 90.0−98.8) and 95.2% (75.3−100.0), respectively. Urine TFV results were associated with short (p<0.001) and medium-term (p = 0.036) self-reported adherence. Overall, 44/124 (35.5%) had viraemia, which was associated with undetectable TFV in those receiving efavirenz (OR 6.01, 1.27−39.0, p = 0.014) and dolutegravir (OR 25.7, 4.20−294.8, p<0.001). However, in those with viraemia while receiving efavirenz, 8/27 (29.6%) had undetectable urine TFV, compared to 11/17 (64.7%) of those receiving dolutegravir. Drug resistance was detected in 23/27 (85.2%) of those receiving efavirenz and only 1/16 (6.3%) of those receiving dolutegravir. There was no association between urine TFV results and drug resistance. Conclusions: Among PLHIV receiving ART, a rapid urine TFV immunoassay can be used to accurately monitor urine TFV levels compared to the gold standard of LC-MS/MS. Undetectable point-of-care urine TFV results were associated with viraemia, particularly among people receiving dolutegravir.
